Anyone tried flutamide? just started.... (Androgenetic Alopcia (AGA) Female Pattern) |
I've been taking cyproterone and spiro for the last 3 years and although I think it did sort of help my hair loss I have a real problem taking these anti-androgens with estrogen.
Anyway, I saw my endo last week and asked if I could try flutamide just incase cypro and spiro was stopping my periods and giving my low estrogen (both cypro and spiro have progesterone activity and can stop your ovaries from functioning via some sort of feedback from the pituitary gland... or something like that). My endo agreed and put me on 250mg flutamide twice a day; I started 3 days ago and have noticed no change (I stopped everything else). My skin is slightly oilier but no acne. I am losing the same numbers of hairs in the shower (roughly 100) but the ratio between terminal and miniturized hairs have changed - I am losing a lot more miniturized hairs and less terminal hairs when I blow dry.
Just wondering if anyone has any experience... I'm really hoping my periods will come back....

Oh, and you said you are losing more miniturized hairs and less terminal hairs. How do you know the difference? I only lose long hairs....

I haven't tried flutamide, just started dutasteride (avodart) a few weeks ago(and am on spiro and desogen). But like everything, I imagine it will be several weeks to months before you will really see any results from it. I think many women think they will see differences sooner, but from what I understand...the whole hair growing process is rather slow as far as turnover and follicles being affected so it doesn't seem possible that in a few days you'd be feeling any effects..it likely would have happened that way anyway. I know...it's so hard to be patient!! I feel like my hair has changed dramatically in the last few weeks and it feels so thin to me in the front. I have alot of miniturized hairs and alot that now poke up all over the place. I lose both terminal and miniturized still, but mostly the thinner hairs that are in the front.

I think this is quite a coincidence....
You, Sadie and I had/have all been taking some kind of Anti androgen and we ALL have experienced the same thing!!! The front thinning and looking miniturized QUICKLy over the course of treatment AND a lot of hairs poking out that are coarser/darker/thicker in diameter) than the rest.
Now, how could all 3 of us have a negative reaction to anti androgens?? Or maybe its not negative but positive?? Maybe our hair is changing to thicker hairs and the ones that were becoming miniturized (thin hairs in diameter) are falling out faster that the new healthy ones can keep up with, thus less density in the front? And notice its the areas most effected by AGA that are changing..
Ladies, what do you think about what I have just said?

I have been taking cyproterone and spiro since stopping birth control 3 years ago and during this whole time I have not had a natural period without taking hormones. I seem to have problems with taking estrogen i.e acne, worse hairloss, nausea (both synthetic and bio-identical). My endo tried to get my ovaries to work via all sorts of hormone medications with no luck ( I just turned 30 so way to young to have no estrogen in the body).
I started taking some form of estrogen regularly about 14 months ago and my hair loss and skin got much much worse which totally didn't make sense because I was taking an extremely high dose of cyproterone (200mg) and spiro(100-200mg). My endo and I tried all these different combos (higher estrogen, lower estrogen, different type of estrogen, lowering the anti-androgens) but nothing worked - during this time all my regrowth came in as miniturized hairs and I now have 25% of my original hair density left. For some reason my body just doesn't like estrogen with cypro or spiro.
So after some research, I told my endo I wanted to try flutamide because for some women, taking cypro and spiro can stop the ovaries from working (both of them have progesterone activity) and obviously taking an external form of estrogen is not working for me... I went to my appointment with printouts of information on flutamide (studies have shown that flutamide with metformin have helped some women to get back their menstrual cycles) and my endo agreed to let me try flutamide for 6 months as long as I get my liver checked regularly.
I have been on flutamide for 6 days. The week before flutamide my average hair loss was 100-150 a day. Yesterday my total hair loss was 77 and today it's on track for being somewhere between 80-90. My shower has gone from 100/130 to 63/65 in the last two days... I have stopped all the other anti-androgens and all I am taking is flutamide and metformin.
I know it is still early but I am hopeful this is going to work. My hair loss has gone from mainly terminal hair during shower/blowdrying to more miniturized hairs which I take is a good sign. This is my last resort - there are studies that flutamide works better than both spiro and cyproterone... hopefully it will work for me...
During my 3 years on cypro and spiro my hair never stopped falling out but I did have regrowth... but my hair was still thinning just slowly.

miniturized vs terminal hairs... |
For me the difference between miniturized and terminal hairs is really obvious. Terminal hairs are hairs that are of normal thickness regardless of length and miniturized hairs are the ones that are really thin and usually quite short. My miniturized hairs are usually somewhere between 1-5 inches in length and are really thin in diameter (sorta like thick spider webs), they are also really soft and wispy.... really different from normal healthy hair.

btw, in my experince growing miniturized hairs or losing hair density without shedding is just bad news. I've always shed hair (there's always been hair all around my house) but I had the thickest hair ever before I turned 26 and I never ever saw a thin miniturized hair in my shower/blowdry ever...
I think regrowth is WAY more important than the amount of hair you shed - if you are losing hair density and you're not shedding more than usual than obviously you're not regrowing and that is just bad bad bad in the long run....

My hair has never shed a lot (even when I was a teen, early adult)unless I have TE whcih has only happened a copule times in my life. SO I disagree that its bad to not shed. I think I would have thinned regardless of shedding or not.

I am having regrowth more than adverage (than when I wasnt taking any orals)..I cant see how trhey could be broken hairs as my hair is pretty healthy..

If you are not shedding more than normal and you have lots of healthy regrowth than I think your hair should be fine... just give it some time. It's only excess shedding and growing vellus/miniturized hairs that is the real problem...

But if you are having a fair amount of healthy regrowth and not shedding a lot, how could someone be visably thinning over like, 2 months? WHich is my case.....Its really strange. The hairs I am growing are darker and atleast as thick as my natural hair. They are tapered at the end so I know its not breakage. I am also having some vellous hair growing in spots where it hasnt before (at hairline that used to have no hair), but overall, my crown keeps getting thinner despite these things. Do you think I should stay on the Diane for a few more months? I cant get into a derm until the end of next month. Like I said in previous bulletin, my hair looks worse than before I started the diane, its thinner on the crown. and its gotten that way pretty dang fast, but yet I see regrowth..... I just need some hope from anyone.

Bunnyheather~I see Dr. Redmond on the 11th and will mention the coincidence. However, for me, I am not convinced the new hairs poking up on my head are thicker, in fact I think they are miniturizing but I'm not certain. They are definitely not thicker, maybe the same if anything. I would think thicker would be a great sign! I suspect my frontal thinning would have occurred regardless because it was headed that way...I don't know that it's a response the the anti-androgens or not or just a pattern for me. Here's what I'm wondering...does one need something extra to thicken up the hairs affected by miniturization or will it happen on it's own once the effects of DHT are minimized and under control? It seems alot of women feel the need to use Rogaine to thicken them up.

If it was up to me, I would definately quit a medication that I THINK is making my hair worse regardless of whether I am shedding or not. Did you ever get your hormones tested before taking diane?

REALLY? Then how do you know you have AGA? maybe you don't even have that. With AGA you get those thin little hairs fall out, that look like they are not done growing yet.

Yes, my hormones were normal. And I am VERY fertile.

I've been on other threads. Maybe somebody can help me. in menopause since 42. now 52. hair drastically thinned starting in November. Probably thinning before but looked good ,but now my scalp is noticeable. Been dealing with going back on HRT, blah,blah,blah. In denial, thought that would fix it. Hairloss dermatologist did biopsy and at my insistence side and back because scalp has few bumps that stayed. AGA diagnosed on top of scalp. had folliculitis on side and nape of nape. on antibiotics. So I said but what do I start taking now to stop hair loss. I forgot to mention he does hair transplants. So that's what he said. Here I thought he was a derm. who dealt with an arsenol of medicines. His answer hair transplant. anway ,He said well you could take Spirolactone. I had tried before. it made me nauseous. Couldn't take it. But he only prescribes 25 twice a day anyway. What kind of hair loss expert is he? Even I know from reading blogs and articles on-line it's 100 to 200 a day, but I can't tolerate it. So I finally got Propecia 1 mg a day from him. Is that okay in place of spiro. I see spiro is anti-andogen and Propecia is DHT blocker. Or should I take sometime in place of spiro and take propecia. I don't understand all of this, but I'm becoming a recluse and am desperate. I started Rogaine two weeks ago 5% twice a day. What do i need to be taking? Help.

Hi Jann
Not sure that a transplant will help with menopausal HL since the hair is subject to diffuse loss due to estrogen decrease. I agree that 25 mg. isn't going to do much.

Hair transplant is not the right option.
Both spiro and finasteride block the effects of androgens though by different mechanisms.
Spiro at a higher dose than you are taking or finasteride can be good options for you.

It really depends on the extent and pattern of the hair loss. FUE hair transplant can sometimes work very well for women hair loss. No knife, no scalpel, no stitches, no visible scarring. Very often 1 session is all that is necessary.
You can see some before and after pictures here. In general, women who exhibit clearly defined thinning areas or bald spots are good candidates for hair transplant.
