Treatment for Pain and Itching (Scalp Pain, Tight Scalp)

I wanted to post this info because I thought it could be helpful to someone.

I have AGA with an AA variant. I was on topical meds for my loss and I added a 2% ketaconazole shampoo to my regime in November. I accidently used it 5 days in a row when I had the flu and it started this terrible allergic reaction in my scalp. I had pain, redness, itching and I had never experienced this before in my life. I went to my scalp derm. and she prescribed doxycycline and unfortunately I had a terrblie allergic reaction to it which I am sure aggravated my scalp condition. Then she gave me
prednsione and doxepin. I find the prednisone helped the pain and the doxepin helped the itching. It has strong anti-histiminc properties. The prednisone was the last resort because my scalp was not calming down. I have been on the pred. and dox. for ~2 months and am slowly taping off the pred. as they monitor the side effects. It has been awful and I cannot use any medication for AGA on my head, but at least I am not in pain constantly now. I have also gained 10 pounds from the 2 medications.

I have also found if I use 0.1% Cloderm + Elidel together at night once a day topically, it keeps the redness at bay until I wash the next a.m.

Also, I have only been using my dryer on Cool or air drying my hair. That has helped immensely and I figured out the small area I always had on my head that never really grew that well, was where I pointed my hair dryer when I used to use the Medium Hot setting. My dr. always thought that spot was AA, but it was from the heat from the dryer!

I hope this info could be useful.

I forgot to say another thing that helped a lot was washing my hair in the sink first with very cool water, then I would take a shower....even lukewarm shower water was causing me to get itchy. Actually, showering first, then sink washing would probably be the best because the head would get cooled down. Soemtimes I have found it helpful to do that at the end of the day...run cold water on my head....no washing....then air drying. I found that washing every other day for a period helped too, but now I do it every day.

And I started oral spiro again to protect my hair because I have no idea when I will be able to tolerate a scalp me. Once I reached 50mg/day, I noticed a big reduction in my scalp redness too.

Thanks for all of those suggestions. I can't see a doctor about my inflamed scalp due to no insurance at the present time (I graduate school in a little over a month, thankfully!) but I have been/am going through the TE process. My shedding has slowed way down, and my scalp is a lot less itchy than it was, but it still gets a little bit itchy sometimes during the day, and it still lookish pinkish and is kind of hot sometimes. I have switched from washing every day to every other, and I now use an all-natural shampoo because I think I was having a reaction to my other one during the TE. Still, I have the inflamed scalp problem. I'm worried that it is a bad sign of something, but for now, since symptoms have been reduced, I'll continue to do what I'm doing and I'll try washing with cool water like you said. Thanks again!

Hi Lindy,

I am glad this helped. I would try to get an Rx (with or without insurance) for the doxepin and see how it goes. Maybe you would not need the prednisone. I do think the biggest factor is the heat inducing flushing which then causes a mast cell release, then the itching, then the inflammation and pain. If I have inflammation for more than 3 days, I get scaling. Right now, with my regimen, I have the inflammation under control and see no scaling.

I use Free & Clear Shampoo (www.nationalallergysupply.com or at CVS) & their conditioner. I would avoid anything with a fragrance (synthetic or natural). I can't use it b/c I am allergic to it, but that can aggravate the scalp and you could become allergic to it during this phase.

I would avoid hairspray or gel too. I spray it on my brush then brush it thru my hair without touching the scalp. It works okay. Better than nothing.

What incident started this?

Ann

Also I forgot to say that I think the Cloderm + Elidel will really help control it everyday. Is there someone who can help you with the Rx cost? Don't let this go untreated. It could linger on for awhile. :)

Thanks, Ann. I think I developed TE because of some health problems and related physical/mental stress, and a medication I took. This was all around August 2006. By January my shedding and itching improved a lot - the itching improved immediately when I switched from the Pantene I had always used, to a synthetic frangrance-free shampoo. But I still have the slight inflamation and itching off and on.

I was at my doctor yesterday and he suggested Nizoral. Every one of the 'scalp treatment' shampoos I have tried made me shed a lot of hair in the shower (Head and Shoulders, Jason Dandruff shampoo, tea tree oil shampoo, and now Nizoral). The Nizoral made me shed more in the shower too, but it didn't make my scalp feel WORSE like all of those other 'remedies'. I will use the Nizoral twice a week as instructed, as long as it doesn't start having worse effects.

I'll give my scalp problem a little longer to work itself out, as it has improved a lot since the fall/winter. And I hope to have full-time and benefits in a couple of months, so any meds or treatment would be covered then. But I now rinse and wash with cool water. And I will see about the injections if things don't improve - do you think the injections or the creams made things better for you? And what was the cause of your hairloss?

Have you thought about seeing your derm for a prescription anti inflammatory? That might help. Normally my scalp hurts but doesn't itch. I switched to another shampoo (has the same ingredient that the other poster's shampoo had) which can cause itching and it does, but no redness and recently, no more scap pain! I was on a pretty heavy steriod a few months ago so maybe that caused the pain to disappear.

anti-inflammatory, and question to nomoresheds

What shampoo do you use? And can anti-flammatories, themselves, cause hair loss at all? Like as a side effect? Or do they only help the situation? Ever since this TE I am leery of any meds I use. My shedding has stopped though (for now!), now I am just trying to clear up the inflammation and try to see more re-growth...

I use tricomin (doesn't cause any itching) and also Dr Proctor's shampoo (which I think DOES cause itching but it's very minor). I don't know if anti-inflammatories can cause hair loss, they can help lessen a shed though for some people. I am trying a variety of shampoos out and I found that nioxin caused a bigger shed..it worried me so I stopped it. It's great your shedding has stopped. How much do you shed per day? Is it only when washing or drying your hair? My hair sometimes doesn't fall out at all and then I have days where it drops onto my clothing all day long. I always lose some when washing and drying.

another question, nomoresheds...

What steroid did you take, and was it strictly for your scalp pain? Also, where can those shampoos you mentioned be purchased, are they prescription? Maybe I will ask my doctor about an anti-inflammatory. It just seems weird that it has been inflamed for this long (since October). Sometimes it doesn't bother me very much, but my scalp will still look a little pinkish and will feel hot if I press down on it. Still, it has improved a great amount since December. I'll see what my doctor says about that.

My shedding seemed to stop suddenly in January - after bad shedding September-December. I had a TE trigger in August. I also would shed during that time throughout the day and during comb-outs and washing. But then at the end of December I went off the pill and braced myself for another TE to strike in a couple of months. In February/March I had a big increase in shedding while washing - but still none during the day. And now that seems to have ended (only lasting a few weeks). I hope that is the last of my TEs! So now I lose about 10-15(?) hairs while showering for the past week or so, which is what it was in January, and still lose only the occasional hair during the day and some during comb-out, which I think is normal for me. I hope my shedding stays down now, and I hope that yours stops! Maybe now that your scalp is feeling better it will improve your shedding.

I wasn't on the steriod for my hair, I was using a strong steriod and antibiotic for another reason but perhaps the two resulted in no more scalp pain. Now my scalp feels great so who knows. I don't remember the steroids name...it wasn't prednisone. It was pretty strong though.

another question, nomoresheds...TO LINDY

I started using Dr Proctor shampoo nano which I buy from amazon dot com. I don't know if it is why the scalp pain is gone, or the meds I took affected it. I still have no scalp pain which is really great. My shed, however, is worse right now. I have a hormone imbalance though so that may account for it.

Emu oil is anti inflamatory and also can stimulate dormant follicles. I've been using it on my daughter's scalp (She has TE from stopping BCP)and she hasn't had any pain, tingling, and her dandruff is much improved. I'm using it on my face at night.

Here are some articles to read on emu oil:
