Making sites for FUE grafts (Hair Transplant)

This is a picture of Dr. Paul making a site. This is at the end of this case. He usually saves a few 1's for the end of most all of our cases to "tweak" the hairline. Both he and Ron Shapiro are very particular in hairline design and approach.

This patient had a typical regression (age 39 years) of about a norwood 2/3 and needed to fill this in and build a new hairline.

The blades we use are fine. They range in a 0.65mm to just over a 1.0mm depending on graft size. This blade was a 0.7mm.

One other thing worth noting is how clean the transplant looks like at this point. This is because of site size and experienced technicians. Some clinics will have the "hamburger helper" look (bloody, crusty, etc) from usage of large blades and punches for that matter.

No craig. Not at any of the clinics I have worked for. We put them into the sites that are made by the physician. Good question.

Here's the thing Bor83... There are many many clinics that do not have a problem with the crusty bloody hamburger helper look in terms of yield. Pretty much no difference... however, in my opinion and many experienced clinics, it is harder to find the sites to put the grafts in when there is larger blade usage. When I came here to SMG I had to adjust to the smaller blades. It actually took time. How are you btw?

Pretty crappy to be honest, but thanks for asking. I'm kind of bitter because I spent about $50K on HT's and my hair looks incredibly thin (and I'm not rich). I had a couple of good years but then when the rest of my native hairs fell out (despite all the meds) it all went to crap again. Worse part is, I realized, and have been told that I got about 60% yield overall - not all the blame from your former employer; the yield wasn't so bad from you guys although can't say anything positive about those 1mm punch scars in my donor. The Double A guy in Toronto did more than a respectable share of the damage with that Home Depot power drill he used to yank out my follicles. :-|

I'm actually considering getting a strip now as it's actually the only surgery option I have left.

Power drills and 1 mm punches both bad ideas cant believe the latest robotic advancement is promoting an even larger punch and people are actually getting into the chair but then again no one informs them. Having a strip sounds like digging a deeper hole but Im not in your shoes so I hope it works out for you.

Hey bor I would suggest going to a doctor that uses a small hand punch, does small sessions and if you have the available donor use body and beard hair. There are only a few that I believe are reputable. Sure its a long slow road but slicing out a strip could easily result in further regrets, just my opinion and I hope you find a solution.

Its also amazing that this robotic fue device is even being promoted with punches larger than 1mm. Some forums almost seem to be promoting and showcasing its use. These guys are not here to help you they are here to make money.

For guys considering a FUE procedure, know this. Mechanized devices and large punches which would describe a 1mm have to do with one thing and one thing only. Its about speed and making money it has absolutely nothing to do with quality and patient care.

If you see a clinic regularly giving young patients very low hairlines that means the clinic does not care about the patient. Why would you go to a clinic that does not care about the patient before you. If a clinic has a few patients that have actually posted thoughts of suicide after having their procedure then you probably want to cross that clinic off your list.

If a strip only clinic tells you that FUE results in low yield they are lying to you. FUE in the wrong hands results in low yield. Why would you go to a clinic that feels its okay to lie.

Most of this is common sense but most want to pretend thats its not there.

Thanks for trying to help but I'm maxed out with FUE. There are too many scars to get most grafts out and I wouldn't want to anyway because my donor looks really thin shorter than a 4 guard.
My first 2 surgeries were manual punches. The doc was the first guy in the USA to dedicate his whole practice to FUE. He trained all these 'reputable' docs all over the world like Mwamba (who worked on me as a trainee) and Bisanga. Results? I would guess 70% yield and thousands of white dots in my donor visible up to a 2 guard. These were 1mm punches. If a FUE graft can't grow well with a 1mm manual punch then when will it ever?

My other surgeries were .75 mm electronic punches handheld by technicians - I call them home depot drills -not those robotic devices that I think you're talking about. Results: probably 60% yield. Beautiful extraction sites - no scarring visible but who cares at this point? And I went to all the guys that said FUE works in the right hands. I'm talking about 2004-2009 so maybe FUE has improved recently but that's now irrelevant to me.

So you see when I buzz my head from 0-4, white dots are visible or my donor looks very thin. The HT hairs don't look very natural either when short- something which is actually common when you have dark hair and light skin because not even expert dense packing can duplicate the density of nature.

Grown out long, it looks more natural but thin as ******* - the kind of thin that people ask "Why don't you just shave your head?"

Bor I understand completely. Its never good to be the first one in the chair and since I have been there done that I certainly wasnt going to do that with fue. Most dont get it regardless of if we are speaking about HM, tattooing or whatever someone that works in the industry is trying to promote you dont want to be one of the first guys out there getting into the chair to see how well it works.

I didnt go to the that guy you speak because not only was he a complete a-hole with a clinic that seemed to be run like sh*t the punches he was using were too large. Thought about one of his techs that went on his own working under a doctor and using .75 punches but just not enough experience or results for my comfort.

Most people cant appreciate that guys like you helped pave the way for FUE to become better but only in the right hands, so thank you. Good luck and I hope the strip works out well and gives you some relief. Sounds like you might have to go for the biggest strip possible in one pass just be careful. I have seen some very low yields in patients that had strip from these so called top strip only clinics and then went on to have equally low yield with fue. I have also seen necrosis recently from a very large strip/fue from another so called top clinic. Much safer to go with a small session but that might not be an option.

Thanks. It would have to be a big strip if I do it. It wouldn't be worthwhile I think to go small. I have great laxity and still have a respectable supply of donor hair.

I remember that tech. He worked on me too- forget his name. They started that clinic in FL with a doc that was just there for legal reasons, if I recall correctly. It's such a shady business. I shake my head now at all the hype over tattooing, Gho, etc. When I went for my first FUE there were already results out for a couple of years so I thought I was ok. Now that I'm wiser I'd say you want a new technique to be around close to 10 years before you can be sure about it. Hey, I still won't mess with my eyes and get that LASIK procedure. I should have approached FUE the same way but hair has a way of messing with you especially when you're younger.

Just curious but isn't this thread about recipient sites and not FUE donor harvesting??..... I think hair tech makes some really good points here. The way SMG creates the sites and implants the grafts minimizes the amount of damage done to the recipient area and ensures that the grafts are seated properly so you don't end up with any skin irregularities. ...
