Donor Area Healing With Acell (Hair Transplant)

Three months ago Dr. Cole extracted 12 grafts from a 1cm area at the back of our patient's head. The donor sites were filled with ACell. A liquid bandage was applied immediately after the ACell application. The photo at the bottom right was taken yesterday (06-11-13) after we clipped the area down. The area healed remarkably well and we could find only four extraction sites out of the twelve.

Donor Area Healing With Acell

The 4x4 photo above shows healing at up to 3 months. In other words, 3 months ago the 12 graft FUE surgery was performed. ACell was applied and sealed under our liquid bandage for the first time. ACell has been approved by the FDA as a regenerative product and at the very least, improves the appearance of FUE donor areas by reducing hypopigmentation.

From the ACell Website bullet point:
"Constructively remodel site-specific tissues where scarring would be expected".

The image below represents aligned photos. The upper left photo was taken immediately after the 12 grafts were removed and the bottom right photo represents the donor area 3 months post-op. The second and third images are the first image superimposed over the last image at varying opacity.

These are excellent photos and show very good healing. Unfortunately, I don't think they demonstrate regeneration as I cannot see any hairs growing at the extraction sites. Again though, the healing looks great.

Great photos and I agree with the poster who said it is easier to evaluate the results from small scale experiments, I can see at least 2 spots which look like regeneration although so far it is only like a black dot. If weekly or daily photos can be taken, then it would be clear whether those black dots are really regeneration or not.

Kindly have a look at 15 graft patch test..where we did invitro doubling and extraction with larger punch,with tattoo marks...there is 100% regrowth and no scar or mark..with 2 months follow up pics...Acell,ecm dp cell and stemcells have power to get rid of white dots of fue...dr cole has proved it ,i have proved with 15 graft patch test..and with other cases in various threads..scarless ht except fut..is already a reality..

I couldn't get Dr. Nigam's before and after photos to align. The hair directions are different and the before and after looks like they are from a different area of scalp. I created a layer with dots marking the hair's exit point from the scalp over the result photo. I duplicated that layer and since it's on a separate transparent layer, I am able to move it around above the "before" photo. I couldn't get the dots to align with the hairs exit points. As I mentioned, the hair direction seems quite different as if from another area of scalp.

It has been 3 months since the grafts were extracted. Many more studies are needed. We have seen follicle regeneration ranging from 10% to right around 70%. The patients we have seen regeneration typically have been examined at 8-14 months post-op.

With this particular study, I began documenting the application and curing of our liquid bandage using our hi-def video camera (Sony nex-fs700u). That is why the first photo has a different tone. The camera shoots 1920x1080 progressive as opposed to interlaced so the resolution is decent. The first photo is a screen grab from a 30FPS clip.

To make a long story short, I have a screen grab from the video camera of the area that is relatively sharp. The best grab I could find is just after two grafts were scored, but not yet removed.

This is the sharpest video grab I could find. As you can see just a few grafts have been scored. I marked a follicular unit with a blue circle on each photo for reference.

kindly also, have a look at the images with tattoo of a 50 graft patch test of invitro hair doubling,even after using 0.9/1mm fue punch for extraction the donor extraction sites are scarless with no white dots..

I can get a close match superimposing the extraction photo over the results photo, but the blue arrows indicate some major hair growth direction discrepancys between the extraction photo and the results photo. Now if you are getting close to 100% follicle regeneration as you claim, post photos that have been taken under tight control standards. Dr. Cole is willing to pay any doctor well who can prove this level of follicle regeneration and is willing to share the technology. Dr. Gho for instance, in an interview expressed his desire to share his techonology with other doctors for a price. Dr. Cole has contacted his office repeatedly and has not to this date received any response.

The brown before photo was taken at my clinic..and under yellow OT lights,the gray pic is independently taken by the forum member in a different light,and with different camera and zoom, after more than 15 days..from his home at london and was posted by him and not me..please visit the thread and confirm the same..how can one graft not regenerate at the grey pic..if it was superimposition..i can understand your doubts..as this seems to be too good to be true..but keep an open mind..may be it is..!
I have not one, but many documented pics..including a present thread of the forum member from france..who is already showing good donor/recipient regen in his early 15 days photos..will post his followup photos,when he comes back to mumbai in july end..we started donor doubling since last 8/9 months with considerable success...
These before/after comparison photos are not posted by me..but by independent forum members gone with hair and antinorwood...in the same thread at hairsite..
I have stopped doing FUE since last few months..i am only offering hair/donor doubling and hair multiplication(an evolving but promising science)with injections of autologous bulge(cd200 and alpha 6 integrin) follicle stemcells and injections of trichogenic dp cells(versican positive) and culture...how would i survive..if i depend on superimposed photos..
i hope, this explanation should clear your doubts regards superimposition ....

Anyway..let me tell you this is an ex hst patient..
You will not have the same experience with me...

I admire dr cole for his work..and would be more than glad to share my work..and i am ready to be tested/scrutinized by any tight control methods..

Hi scarred5:

By revision, if I'm understanding your question, you mean cutting away scar tissue and closing the wound. Acell does help in some cases but often times it's a tight scalp that causes the scar to widen and it could cause a strip scar to widen back up. Dr. Cole is having great success grafting into scar tissue and ACell improves the yield.

As a patient myself, the concept of doubling up the follicles is indeed exciting. I'm looking forward to seeing a meeting happen between Dr. Nigam and Dr. Cole. I doubt that Dr. Nigam would be willing to meet with Dr. Cole if he weren't really on to something. They are already building human organs in the lab using ACell, so we're not talking time travel or teleportation. The concept seems far-fetched though and we all need to keep an open mind.

There is often plenty of donor beard hair below the jawline. The next best hair source is chest hair. Are you happy with your recipient area?

As far as I can tell, beard grafts in scar seem to work very well, right now that is my number 1 choice for my strip scars, and if everything fails, scalp micro pigmentation will be my last resort.

results vary, ive seen some incredible results and i know first hand, cause ive had one of my scars implanted 4 times and unfortunately part is still visible even with my hair longer, which is why im not sure i want to attempt it again, ive had some success with revision, my last one with dr bret bolton, in florida, some of the best scars ive ever seen, depending on the size and location of the scar, a revision to reduce prior to implanting hair might be an option, ive been in contact with cit, who has been very helpful

How long ago was it when you had the revision done? I wonder if the scar will stretch again over time, to me revision seems to be a temporary solution, it buys us some time until scalp laxity becomes an issue again.

i had several revisions done over the yrs with wolf, rahal and recently, with dr bolton in florida, who has the best scars ive ever seen, all resulted in improvement, im thinking of going again for a small partial one, but prior to having each one i would work the laxity of the area to be revise, by sqeezing and pinching, but then again i had more than one scar, i think mine have improved over time, but i agree thats not always the case, i would also recommend no gym for 3 months, im now trying the dermaroller

why dermaroller? isn't this the one with needles? i used it before to enhance absorption of minoxidil and other topicals but this is the first time i heard people using dermaroller for scar repair.

Dr. Cole uses a dermaroller when administering PRP. I have heard that it can be used to improve the appearance of hair transplant related scars. Currently only physicians can purchase ACell. I'm curious if an ACell/PRP mix could be beneficial, applied to the scalp and micro-needled.

If you do a Google search for "derma roller hair transplant scar" without quotes, at the bottom of this forum thread you can see a photo of a guy who used a dermaroller on his strip scar.

yeah thats why i use it, people actually use it on their faces,so i dont think it would harm or make things worse in the donor, i just wonder if one gets results from prp, that results may not last and you would have to do repeat at regular intervals, just not sure but would love to hear the results you get if you try it

In response to your post, I spoke with Dr. Cole this morning about PRP. He feels that PRP mainly helps with healing and improves hair shaft diameter. Outside of hair transplant surgery, PRP has a proven track record of improving healing in patients with injuries such as those that occur playing sports. Emory Healthcare for instance offers sports medicine training in the use of PRP. Dr. Cole just showed me a study of the effectiveness of PRP in hair transplant surgery that is currently underway. The study solicits data from hair restoration doctors about hair regrowth after PRP treatment. Dr. Cole personally hasn't seen hair regrowth but rather an increase in hair shaft diameter in patients treated with PRP. Cross sectional trichometry has determined that PRP does indeed increase hair shaft diameter in some patients. At the very least, PRP improves healing. Also, the benefits of PRP can only be maintained by regular treatments approximately once per year.

do you think prp would be of benefit in a scar revision? i remember a few yrs back jones used it on wound not sutured shut, and i believe it still healed quite good, not perfect but surprising well, do you remember it?

Dr. Cole has seen great results from using a combination of ACell and PRP. One hair restoration physician in an interview expressed his frustration related to doctors not getting satisfactory results by using ACell because they are simply not following directions for use available from the manufacturer.

The following is a photo I took yesterday of our RN next to our "Angel" device that we use to create our PRP.
