Dear forum readers,

The patient TH sent his 5 month update pictures. He is pleased with his progress thus far. Pictures seem to be taken with wet hair.

Patient particulars -
Norwood 6.
Of Maori descent.
No previous hair transplants.

No grafts were transplanted in the crown area in his first HT.

Hair graft particulars -
Body hair grafts (2584 grafts)
- Abdomen : 126 grafts
- Armpits : 206 grafts
- Chest : 719 grafts
- Legs : 1533 grafts

Dear JonnyE,
This was the first stage of his hair restoration.
Further work on temples, hairline and crown remains to be done in future.

For the number of grafts in the 1st HT, hairline placement, area covered, patient's preference and naturalness, all were factored in.

This is surely one of the wow results; a dramatic change and only a couple of thousand scalp hairs used and only 5 months out. Thanks for clarifying what was and what was not covered.

Did this guy have exceptional body hair?

Do you have a reasonable idea relating to the predictability of BHT?

Dr. A, you use an apparently novel concept of the expanding needle. In a prior discussion you mentioned multiple stabs at a donor extraction site. Maybe I misunderstood but I am trying to get a grip on the size of the donor excision since if I decide to go outside Europe I would like to be treated at your clinic and want to get a feel for this issue.

At the risk of being condescending, I think that your progress over the years has been Stella.

My apologies for the delay in reply.

No, I will not call TH's body hair exceptional. Average, maybe.

I have shared my views about BHT, based on our follow ups on threads before.
If you wish, I will request V to compile links to some of the recent threads.

Expanding needle concept is simple. We have tried to explain this before and have shown it to many of our patients.

I will have another go at it.

Think. The problem most doctors face when extracting from areas like forearms, thighs, legs etc., using a punch in their hand is the laxity of the skin.

The laxity leads to the skin (and the follicular unit) getting distorted when they twist their punch. The skin twists alongwith it.

That leads to the transections.

Now think this. When you poke with a needle, the skin doesnt turn/distort. But it gets cut.

Imagine cutting around the FU. That is the first/ one variant step of the expanding needle concept.

Instead of using the punch in hand, you just do it a different way.

I hope I have answered your query? Do not hesitate in case you have further doubts.

Dr. A thanks for the explaination. That is clear to me now. The fracture force at a point is indeed massively increased compared with an edge no matter how sharp. I can think of configurations that would allow the use of this concept (without going too deep) with a single placement but ofcause manufacture is always an issue.

Stella should read Stellar as bellow. :-)
