8 months post op the patient send us update pictures.
Please note that the patient wanted to keep his existing hairline.

Those pictures are better. They represent a better idea of the result versus the previous photograph, but I can still tell the hair has been styled for the photo in a manner to depict the best coverage.

The first before photo makes it clear that a good amount of hair in the frontal area, after, is pre-existing hair. The pre-existing hair in the before photo was clearly just not styled.

This is another example of why it would be good for people to post before photos of the area wet, and then after photos of the hair wet.

How about a picture in the SAME lighting and at the SAME angle as the before picture? It's very easy to take a picture at a certain angle and in darker lighting and make the hair appear fuller than it actually is.

I have to agree with ppz. The result of the HT is a tuft of thick hair at the front but from oblique angles this looks an unatural distribution compared with the vertex and crown. i do not think this can be described as a stand alone procedure. If there were enough grafts to continue the Ht throughout then the approach would be good but there aren't.

Even though its the patient right and wish to do whatever he wants to do, its also your responsability to let him know that putting so many grafts in such a small area could not be a good idea both in ST, MT and LT.
Even though you mentioned that he is getting additional coverage done to cover the big area that remains, already you see that there is something wrong or out of shape when you see the photos, its like he had something artificial stuck over there.
I think the hair line looks good and the result is also good, but feel there is too much area and doubt he might have enough donor to obtain a natural looking result. If he does indeed have, then he probably will have obtained what he wanted.

I do agree that hair line even receded will probably have a higher density but this man is NW5 and you are saying the same number and maybe less will give anything like a coverage with any density? I hope he does not expect full cover and decent density because he will not be happy

It might be hard to understand to some young readers that some patients intentionally do NOT want a straight teenage hairline. We give the patients the choice of the design. This case is a perfect example of giving the patient the hairline HE desired.

Typically transplants have higher density in the front, lower density towards to crown. The top of the scalp (hairline to crown) is easily 140 cm2. The amount of grafts needed at high density to cover a bald area would be too much and we have not even touched the crown.

BTW, we have patient that opt for lower density, more coverage. Most of them come back for more density however.

I think I would have opted for less density, more coverage. Do you have patients that do that?

Also, I don't understand how he can cover the rear with 3000 grafts. He had 3000 put on what appears to be less than 1/3 of his thinning area. How will he cover the remaining 2/3 with the same amount of grafts? Will the rear be thinner or are you saying the rear as in a bald spot that needs to be filled in?

From that first pic it looks like the transplanted hair doesnt blend in that well with the existing hair.

The out door pics look good and the placement tidy; but you seem to have left him with a tuft of hair at the front and thinning behind also it doesn't look like you joined the sides up to the grafted area. How many more FUE grafts can he have if and when he loses the rest?

The area that was transplanted looks thick but i think he needs to have the temples filled in.
