Dr. Woods: Repair of Repair of Pluggy, Uneven and Scarred
Hairline with FUE. This is a repair case with 2000 FUE grafts. Dr. Woods
soften the pluggy, uneven and scarred hairline of this patient after 3
sessions. The repair was done by utilizing nape hair, chest and
abdominal hair. None of the prior pluggy grafts were removed in the

Dr. Cole: Body Hair Transplant Megasession 5000 Grafts on the
Crown Grown Out Results plus Amazing Leg Hair to Anterior Temples 2.5
Months Results. This is the first body hair transplant megasession grown
out results posted in the forum. The patient had about 5000 BHT grafts
placed into 80 square cm of bald crown. The result is exceptionally
natural with very respectable yield. This discussion also features an
attempt to lower the patient’s hairline with about 1700 grafts of leg
hair as well as a 2.5 month post op results from leg hair transplanted
into the anterior temples.

Dr. Woods: 5,500 FUE/BHT grafts, 4 Weeks Results, Grafts Started
Growing Immediately Post Op
. 4 weeks post op photos of a Norwood 6 patient who had received 2,500
scalp donor hair and 3,000 chest and abdominal/loin hair. Total of 5,500
grafts. The last session was 10 days ago to the frontal hairline. The
scabs were removed immediately prior to these photographs being taken,
hence the minor pinkness to the frontal hairline. Abdominal hair was
placed to the crown 2 weeks ago prior to these photographs. A close up
shot is provided and some of the scabs are still present. There is good
growth right from the start. All graft hair shafts were 1 to 2 mm in
length. As is clearly seen, many hair have extended by several
millimeters since the procedure.

Dr. Arvind: Combination of a Dense Packed Hairline with Hair

Method (DEM), over 1000 Body Hair grafts used in Donor Site.
Multi-modules treatments and HairSite’s first documented case of Donor
Exchange Method utilizing over 1000 body hair grafts on the patient’s
donor site. 1.Closing of the temples- 2628 FUSE/fue grafts used in
the front hairline and temple areas to create a dense pack hairline as
well as to close the temple angles. 2.Donor exchange (DEM)-
over1000 body hair grafts used for putting into the scalp donor

Dr. Cole: One of the Best Crown Restoration Results with Only 700
grafts. This Norwood 5a – 6 presented his case to Dr. Cole back in
1997. At that time Dr. Cole was one of a small group of hair restoration
surgeons performing total microscopic follicular unit hair
transplantation. The patient followed-up with two more surgeries in
early 2004 – one a microscopic FUT strip session for the frontal and
mid sections and the other a 700 graft FIT/FUE procedure to address the
crown. This is one of the best documented case of crown restoration

Dr. Arvind: FUE Donor Sites, Up Close & Personal Using Donor
Sealant Technique. This discussion focuses on the
appearance of the donor areas where Follicular Unit Extraction (FUE) has
been performed and the Donor Sealant protocol has been followed.
Different scenarios have been presented:
1. the donor area has been
shaved (hair being 1 mm or less)
2. the grafts/FUs have been densely extracted. 20-30 FUs have been
extracted per sq cm.
The following issues are
discussed:
1. Appearance, or otherwise, of hypopigmented dots.
2. Appearance, or otherwise, of empty spaces but no white dots.
3. Contrast with the hair density in the surrounding areas.
4. The distance from where any anomaly (dots, spaces, contrast) is
visible and the distance at which it stops manifesting itself.

Dr. Cole: Results of 400 FIT/FUE grafts on an African-American
patient. This patient’s pictures were taken one year after his initial
procedure of only 400 FUE grafts into the temple areas. Here is a
classic example of the “less is more” phenomenon and it also shows
FIT/FUE results on a African-American patient. These grafts were
extracted via Dr. Cole’s FIT/FUE method and, as usual, they have
matured beautifully and healthily. This is a good example of what can be
achieved when excellent hair characteristics meet ultra-refined surgical
techniques. A former Norwood class 3a, this patient now enjoys a more
flattering but still age-appropriate hairline.

Dr. Arvind: Repair Patient with 2300 FUE grafts, 11 months Post
Op. A repair patient who had 2300 FUE grafts 11 months results. This
patient has some mini grafts or plugs in the temporal recession areas.
Grafts in these areas should ideally be excised but the patient did not
want to touch them at the moment. Dr. Arvind’s team, therefore, only
concentrated on the front half of his scalp. These pictures are taken
when the patient came to Dr. Arvind’s clinic to have the grafts
excised as well as to receive another 500 FUSE/FUE grafts placed in his
crown area.

Dr. Arvind: Repairing Minigrafts in the Temporal Areas with FUE
Techniques. This is a patient who developed traction alopecia- loss of
hair resulting from constant pulling and pressure exerted onto the scalp
and hair.
He was previously administered rows of minigrafts and plugs in the front
hairline and the temple areas by another doctor. Dismayed with the
results, this patient convinced the original doctor to excise the plugs
in the hairline. The plugs were excised in form of a strip which was
then sutured closed. The temple area minigrafts, however, remained until
this patient visited Dr. Arvind Poswal.

Dr. Arvind: How Donor Sealing Works in FUE Donor Sites? The
reason behind the success of donor sealing is that 1. It returns the
pigment bearing cells back to the donor sites where they are required to
avoid appearance of 'white dots'. 2. It helps in promoting healing
of the extraction sites by making "healing by primary
intention" possible.
In the FUSE extraction sites, since the wounds are round and close and
many in number, traditional methods of healing by primary intention are
not possible (except in the donor exchange method). Therefore, the
extraction site will heal by collagen formation from the dermis upwards.
This collagen is white in color and is responsible for the "white
dots".
Donor sealing creates a virtual seal of pigment bearing epidermis on top
of each of the extraction sites. The surrounding epidermis, joins the
donor sealant to provide "healing by primary intention".
The deeper dermal layers do heal by secondary intention and do form
collagen, but the pigmented epidermis on top of the extraction site
makes sure that the collagen tissue remains below and is not visible.

Dr. Cole: 900 FIT/FUE
Results into the Top Mid-section of the Scalp plus Plugs removal. This
is a 900 graft session of CIT (FUE), the Cole Isolation Technique, to
the top mid-section of the scalp. The hairline was not grafted. Rather,
Dr. Cole began removing several large, "pluggy" looking grafts
from the right side of the hairline. The crown was not grafted either.
Patient was quite satisfied with the healing of the right frontal hair
line without scarring following tdäèüäpÌÁal of several of the very
large noticeable 2, 3, and 4 hair grafts that were on the hair line. The
patient now returns to Dr. Cole to remove many other large grafts from
the hairline and to build a new hairline that is softer and more
natural. this will blend well with the outstanding coverage he obtained
from his relatively small first procedure.

Ever Donor Exchange method Performed on Topjimmy. The Donor Exchange
Method appears as a logical course to follow for those who : 1. People
who do not wish to use body hair over the bald recipient sites but have
a degree of hairloss that is too severe to fill with the scalp donor
hair alone. For these individuals this avenue opens the possibility of
harvesting the scalp donor areas to a greater extent while still
minimizing chances of visible thinning in the back of the scalp. 2.
People who may have very thin and fine body hair growth. These
individuals may like to use the very thin/fine hair in the scalp donor

Hair Transplant Plugs Without Excising the Grafts. Repair of a wrongly
performed hair transplant involves decision making in equal parts on the
side of the doctor as well as the patient. Sometimes, the patient may be
wary/unwilling to follow certain approaches that the HT doctor may feel
will do good. In such a situation the patient may have a plan of action
in his own mind. Some such approaches may be beneficial, others maybe

This patient had 900 grafts (Feb 04) placed just shy of one year ago

Sealant Protocol, Step by Step Illustration of the Exact Methodology.
This method entails removal of the epidermis from the graft. The
follicular unit (FU) is then placed with the hair just visible on the
top of the recipient site. According to Dr. Arvind, the superficial
epidermis is not required for the graft growth in the recipient area.
However, it is helpful/required to give good results in the donor area
and reduce signs of visible scarring. Dr. Arvind and his team have been
performing the donor sealing technique for scalp as well as body FUSE
donor areas for over 7 months now. They have not encountered any side
effects and the results in terms of minimizing any visible scarring have
been extraordinary.

Repair Procedure of Over 3900 grafts from the Hairline throughout the
Top and the Crown.
This is a patient who had multiple plug grafts placed several years ago
by the old punch graft method that left shotgun scars in the back of the
scalp. He was a class 5A to 6 and all his hair from the top to the crown
area was completely bald. He also has numerous elliptical scars in the
back of the scalp where someone shamelessly talked this person into
suturing a hairpiece to the top of his scalp. 13 months ago he underwent
a FIT/FUE procedure of over 3900 grafts from the hairline throughout the
top and including the crown. The old plug scars in the donor area
reduced the total follicular density so that Dr. Cole’s team had to
harvest follicular units between, above, and below the shotgun scars.
The old hairline was too low and the hair growth angle was incorrect and
unnatural. All of these factors required a complex repair plan that was
carried out 13 months ago at Dr. Cole’s clinic. Dr. Cole and his
team also added some FIT grafts on the "shot gun scars" in his
donor area. The plug scars and other scars are not visible
anymore.

Case for Patient with Low Follicular Density Made Possible by FUE
.This patient is a repair case who desired additional coverage in the
crown. He had numerous procedures in the 1980s including plugs and scalp
reductions. This left him with a pluggy hairline and pluggy tear drop
shaped crown. Tear drop shaped crowns result from midline scalp
reductions that lower the crown into the donor area and elevate it
toward the front. He did not want to soften the hairline. He simply
wanted more hair in the crown. His donor area was extremely tight and
diffusely scarred with a low donor density. Dr. Cole and his team
attempted one strip procedure on him, but this yielded only 400 grafts
due to the low follicular density, excessive scarring, and extremely
tight donor area. Subsequently, Dr. Cole was able to remove 2000 grafts
with the FIT/FUE technique. This is 7 months post op results.

Scar, 300 grafts, 8 months Post Op. This patient had a procedure 8 month
ago. He had 300 FIT/FUE grafted on his scar. The work results are
fantastic. Noticed the pinkish color in the before pictures, it has been
dramatically reduced in the after photos. According to Dr. Cole’s
team, they have noticed reduction in color with some patients that had
FIT/FUE grafted into a scar, and it is definitely helping along with the
growth to the overall appearance of the donor. The growth is excellent
and the patient is very happy with the results. Even more growth will be
expected at 12-15 month's post op.

Zone Harvesting: Sample Pictures. Donor site pictures of a patient who
recently underwent BHT about 2 months ago. The donor follicles
were extracted from the patient’s armpit, abdomen and pubic
areas. These pictures are taken when the patient shaved his armpit
2 weeks back. The healing in the so called “wet zone” areas was
noticed to be faster and with NO scarring as compared to the chest. No
complications noted.

class 3 before his 2000 Grafts surgery on February, 2004 all over the
front of his head. He was a virgin scalp and this was his first

from his scalp donor area. His hairline was dropped about 2.5 inches
down on his forehead. The most noticeable improvement was in the massive
areas of complete baldness in the temple regions. He is extremely happy

5041 BHT with Dr Arvind 1 Month Update. “I had 5041 BHT all from my
legs with Dr Arvind in New Delhi. I have shed the majority of my grafts
and unfortunately the picture doesn’t show the full story. I have a
large number of tiny hairs sprouting and I can feel stubble over most of
my scalp when I rub my fingers lightly over it. The redness on my scalp
is not noticeable to the naked eye, and from a short distance it looks

sealant to all the chest hair donor sites. No visible scarring can be

grafts removed by a 0,75 mm punch. He is lucky to have very long hair to
cover his shaved area and went back to work 3 days after the procedure.

Dr. Arvind: Pictures
of Bruce_DC who had 2500 FUE performed sometime ago. The pictures
presented here are of the donor area where Dr. Arvind performed the
Donor Sealing protocol. According to Dr. Arvind, donor sealing greatly
improves the cosmetic appearance of the donor site. Bruce_DC had also
gone for a small "test" session earlier (about 6 months back).
Dr. Arvind recorded a greater than 95% yield in those previously
transplanted FUE grafts. (77 out of 80 grafts at 6 months). Pictures of
that area are also included here.

Dr. Cole: Latest Update from DJ, One of the Worst Repair
Cases Ever. DJ is one of Dr. Cole’s repair patients. He is by
far the worst repair case I have seen in person. Dr. Cole had to
deal with a head that has gone through 13 small strip surgeries of 50
grafts each, 3 scalp lifts that produced 3 huge scars down the center
all the way to back, 4 scar revisions from ear to ear that caused even
wider scarring, and last but not least, tattooing all over the head that
eventually turned his scalp color to blue. We are showing the part where
Dr. Cole grafted the ears and the neck area that had almost no hair.

Dr. Cole: These are pictures of one of Dr. Cole’s patients who had
a mega-session of almost 5800 grafts (4800 BHT+1000 FUE) 14 month ago.
His crown have excellent growth from the body hair and what’s more
amazing is that there is much more to come. According to Dr. Cole’s
team, there are little grafts growing on his whole crown even after 14
month.

Dr. Cole: Repair Case – Strip Repair Case, Dr. Cole’s
Patient. This is a class 6 repair case. The patient used to
wear a hair piece. He had one session of mini-grafts placed into holes
in 2003. He could tell immediately that this procedure would yield
an unnatural, pluggy result so he rapidly sought a repair
solution.
Dr. Cole performed 3000 grafts from the front to the back. In a single
procedure he has obtained excellent coverage in the front. The
rear of the scalp needs one more session (the back or crown almost
always requires 2 to 3 times as much work to achieve the same degree of
coverage). Most of the grafts were dense packed into the front.
Dr. Cole and his staff also needed to deal with big plugs and the
results are showing a softer natural look. The patient can live a normal
life without the hard maintenance of a hair piece and he is very excited
about his 2nd procedure.

Dr. Arvind Poswal: Hyper-acute Angle Placement for Hairline
Restoration. For some patients with extensive thinning, eg: a
norwood 5 or 6, Dr.Arvind advocates the formation of recipient sites at
a much more acute angle for such patients. Instead of having the
hair growing at 30 degrees to the scalp, they almost lie flat,
i.e.,15-20 degrees). According to Dr. Arvind Poswal, the rationale
for such an acute angulation in hairline placement is that it makes the
hair almost lie on the scalp leading to a greater degree of coverage
than would be possible if the hair were, comparatively speaking,
standing up. Dr. Arvind Poswal thinks that hyperacute angulation
is especially helpful in those with extensive hairloss and a limited
donor supply.
