Nobody said it is not important, it's just kind of difficult to estimate how many grafts I need and unsure who to believe. There are a lot of good pictures in the forum, the only problem is consistency in no. of grafts, sometimes 2000 grafts look fantastic on someone but look so-so on another poster even if they have more or less the same norwood pattern.

That is because he had 2 surgeries, as stated in the opening statement. The first is a 3000 graft surgery done 2 yrs ago. The second surgery is ~1600 grafts done 1 yr ago.

On another note, in another post you state you are my patient. All my patients have a follow-up regimen that should be adhered to. If you have not followed up with pictures and updates, kindly do so as soon as feasible.

Those pix look like a great transformation. Does hooray for hair still post on here? I'm not going to Austrailia anytime soon, but would like to ask him, about undetectability. does it look "fishy" in anyway or does it look 100% real, no questions asked perfect normal hair in every way.

Good question.

In the instance of HFH, there is abscence of non head hair (genetic). Based on this limitation, the present hairline is the most aggressive recommended for him. ALl future hairloss would be catered to using head and donor hair. He has at least 3500-4000 of these grafts in reserve. Given his age, I estimate this to be enough for future needs if any.

What if this guy goes to a NW7. the most recent transplant you gave him
in retrospect, looking back if he were to go to that severe level of thinning, would be viewed as way too aggressive, and used way too many of his available grafts for far too small an area.

If someone were a NW7 would you transplant over 4000 grafts into the area you just transplanted on this guy? I think it would be considered foolish, since the rest of the scalp would be severely thin, in comparison,

\what if the sides from the part line down, and from the crown back, were as severe as most NW7, the frontal hairline you just planted would look ridiculous at that point

dont you think if guys were for whatever reason, to recede to this point, of severe hair loss, after having received such a thick frontal top , that they would be basically out of luck at that point> and not be able to fix their hair with the remaining grafts"?

the main what if is
what if this guy goes to a NW7 then what?

he used up 4000 grafts on about 30 percent of the front top, he is basically screwed if that happens since the previous transplant is way too thick to evenly disperse his available grafts over the bald area

its like you have 50 grande in the bank to last you 10 yrs and you use up 20 grande in one month

not too smart

I would love to hear an answer from a doctor about this possibility of severe loss in future, and what can be done about it

hopefully the answer will not include ...he can use body hair/../ body hair seems to be very expensive and unpredictable at this point

"What if this guy goes to a NW7" ?

This is a good question that any patient and transplant doctor are advised to heed during at the planing stages.

1. Given his age, this is highly improbable

2. of the 4600 atleast 2000 came from non traditional donor areas (nape, around the ears temples). About 2600 grafts from the traditional head donor is not too much to use in restoring and reinforcing the entire hairline and frontal scalp and aspects of the midscalp.

3. His hairline is less than a NW2+ that has been designed to look more aggressive than it actually is. It is a cautiously placed hairline. The second surgery did not advance the hairline from the 1st. By comparison, the motorcycle rider in the following video has an aggressive hairline. Dr. Umar, do you recommend for new patients to incorporate head, body and beard hair from the very beginning even if the patient seems to have enough head donor at the time of the procedure. Beard & body hair can add so much more density without wasting precious head donor.

Hello Dr and thanks for the reply
I think my next question would be, how do you determine the risk for future thinning? \

I guess it would be a combination of patients age, family history, and the length of time he has been losing his hair?

I know you cant just go by patient age. Look at Letterman, He is in my opinion a NW 4 to 5 but he has lost most of this after the age of 50, since he is now 62

i have seen many transplants
this one does not stand out as detectable

most of the docs on the board, Umar, Dr A, etc etc
are so far past the skill level of "obvious transplant" that its not even worth discussing

there are many docs who are not at that skill level yet but I would say the vast majority of docs on this board are vastly superior to the point their work can easily be detected

Actually Natural is correct in his observation. It stands out as a hair transplant for sure. Hence the reason for no close-up of the hairline;-)

other than that I think it's a fairly decent result for the coverage recieved.

Many ignore the fact that the donor site thins too!

4000 early on in the balding process may be 500 hairs later on.

Look at a 60 to 80 year old's donor, usually very thin. These are the same hairs that get moved up front... they thin just the same!

No one "in the business" wants to admit this fact, but it is reality! My donor density and transplants are at about 50% of when I had procedures about a decade ago!

Each patient has to be approached differently. Recommendations ought to be customized according to factors that include but are not limited to:

1. Head donor availability
2. potential future needs
3. Patients preferences
4. Suitability of non head donor supply.

I've been thinking about the same thing, I've never head hair transplant before. If I add some body or beard hair to the mix, this will surely reduce the demand for head donor and increase the supply of total donor availability. As long as we do not use body hair for the hairline I really see any harm with this strategy.

Hi Fast,

Actually, Dr. Umar used leg hair to "repair" my old HT hairline and about 1000 grafts did the trick very nicely. My leg hair blends perfectly with the 1990s "micro" strip grafts growing behind my new hairline.

IMO, nape hair and yes, sometimes body hair can indeed be used in the hairline to create that "soft" transition zone just before the stronger, thicker head-hair grafts that provide density.

Results shown are from pictures received from Hooray for Hair. All of what was received was posted. So at this point I would be unable to meet special requests. You have also stated that close-up pictures of the hairline were not posted in order to obscure a transplanted look. This is not the case. We have no control over pictures taken in Sidney Australia. For some reason I am, once again having trouble uploading pictures directly to this site, (even though my pic is a jpeg) so if you click on the link it will take you to directly in my blog what you seem to be wanting; a close up of the nape area (back of the neck) where grafts were extracted. If you click on the picture it will give you a close up.

These pictures of the nape area where grafts were extracted were taken only 2 days after my procedure which was performed on October 11, 2007.

When the term "nape hair" was used I was always under the impression that it was the hair from the back of the neck area, not the bottom portion of the donor site (lower back of the head). I thought it was refered to the area us men always shave off when we get a hair cut on the back of our necks.

This is not nape hair, this is the lower portion of the head. Nape hair is the hair on the back of the neck, the hair we always shave off.

So when surgeons say they used nape hair, they're using the wrong terminology? Because This is NOT nape hair, unless keepitshorter showed me wrong pics.

Dr. Umar can you clarify this pls? This is why I was so interested in seeing the pics post op of the nape area to see what the area would look like as this would always be visible.

If you feel the back of your neck you will notice that there is in fact hair that grows out of the back of the neck right where the top part of your neck begins and your skull ends.

From my understanding this is the nape of your neck and hair does grow from this area and can be used, as in my case.

Your donor extraction is from the back of the head and circled in your pic.

I don't want anyone thinking I'm slamming the doc or these results. The results in my opinion are just fine. I'm only discussing where the nape area really is, because this entire time I was imaginging the back of the neck. Hence the reason for me always asking for pics of the area.

The description of these surgeries indicate the extraction is from the nape area (nape hair). Which isn't the case.

The nape area is circled in red your pic and there is no extraction from there.

The back of the head is never called nape....the back of the neck is nape.

Red = Nape hair/ Nape Area

Black = Back of the head; extraction sites. AKA back of the Head.

The bottom portion of the black area you have circled towards the right is where the SKULL AKA HEAD ends and the NECK area begins and grafts were taken from that area.

In the second picture of that part of my blog showing grafts taken from the right side, which you did not use as an example, clearly shows many grafts were taken from the area WELL below the earlobe which is the NECK area not the SKULL AKA HEAD area.

Yeh on this side there does seem to be a few grafts taken from the nape area.
I think the picture is cutting some off.

I think that is a smart approach, there is really very little or no downside IMO. Nape and leg hair should work very nicely for the hairline, the worst that could happen is that you get very poor yield.
